Send
Person's name (will be printed on the calendar)
Time, date and place (city, state, country) of birth
City, state and country for which the calendar is to be calculated (where the calendar will be used)
Month and year the calendar is to begin (usually the current or next month of the current year)
Postal address where the calendar is to be mailed
$15 (cash or money order in U.S. funds only payable to Mantra Corporation. Price includes postage and handling. Add $4 for calendars to be mailed to addresses outside Canada and the U.S.
